Case Study 4
Indonesian - Blocked tubes, male factor, IVF_ICSI, triplet pregnancy


Mr and Mrs C, both in their mid thirties, were high fliers in their career. Mr C was based in Indonesia and Mrs C in Singapore. Due to their hectic work schedules, they only managed to get together for 4 months each year. When no pregnancy was achieved after 4 years of marriage, Mrs C approached doctors in Indonesia where she realized that she had blocked tubes.

An unfortunate natural disaster happened and Mrs C lost many of her relatives. The fear of this prompted Mrs C to seek help. After several contacts with a few IVF centres via the internet, she decided to seek help at Thomson Fertility Centre.

At TFC, her husband was diagnosed to have some sperm problems as well and IVF/ICSI was recommended. Mrs C started her IVF treatment in June 2005. She had 8 days of injections of Gonal-F which stimulate her ovaries to produce eggs. A total of 4 eggs were obtained and ICSI (Intracytoplasmic sperm injection) was performed to increase the chances of fertilization as the sperm count was low. Three of the four eggs were fertilized thereafter and the three embryos were then replaced in Mrs C’s womb.

Two weeks later, Mrs C’s urine test showed that she is pregnant. Two weeks later an ultrasound scan at 6 weeks of gestation detected 3 sacs with fetal hearts!

Mrs C eventually delivered 2 boys and a girl.

Glossary
IVF
In-vitro fertilisation. A process where sperm are placed together with eggs extracted (by a minor operation) from a woman in the laboratory culture dish to enable fertilisation to take place.
ICSI
Intracytoplasmic sperm injection. A procedure whereby a single sperm is injected into a single egg by means of a very fine needle using micromanipulation techniques.
MESA
Microepididymal sperm aspiration. A microsurgical operation where sperm from the epididymes (part of the testes) are aspirated by a fine needle.
